This is a pressure sensor built for the Cummins ISC and ISL platforms, both common in medium-duty trucks, buses, and some fire apparatus. Depending on where it's mounted, it's reading oil pressure, fuel pressure, or boost pressure and sending that signal to the ECM so the truck knows what's going on inside the engine.
These sensors don't fail dramatically most of the time.
