It's LH only, so if you need both sides you're ordering the passenger unit separately, part number F01159-R if we've got it listed.
Peterbilt built the 388/389 series on basically the same front-end sheet metal, and the 367/567 share a lot of that cab and hood architecture too, which is why one headlight assembly covers all four models. That said, Peterbilt made running changes over the years - different mounting tabs, different connector plugs on some model years - so this isn't a blanket 'every 388 ever built' fit.
Surest way to know for sure: pull your old headlight and check the Peterbilt part number stamped or on a sticker on the housing, or just run your VIN. Don't guess off the model name alone.
